Request for Company Protocols

Dear Sir or Ma’am-

In an effort to keep all of our Angelicum family and partners safe and healthy certain measures have been undertaken to prevent and mitigate the possible spread of contagion of the coronavirus. As part of our protocols, developed in conjunction with the Risk Assessment provided by an independent security agency, we are requesting a copy of your company’s protocols to be filed in our administration office.

We kindly ask you to review the attached External Workers Safety plan and ask you to share it with your employees so that they are aware of the protocols necessary to obtain access to the facilities. Additionally, you will find that we have attached an acknowledgement form, to be signed by an officer of the company, ensuring that your employees are aware of and will adhere to the protocols for External Workers while on our campus.

A copy of your company’s protocols can be emailed to: [email protected]. The Acknowledgement form must be signed and the original returned via mail or dropped off at the reception desk in an envelope to the attention of Administration Office/EWA. Please submit these documents to us no later than 30 September. Failure to submit these documents may result in a denial of access to the facilities.
